Convert standard output renderer callbacks to hooks

      Note: Unit tests cover most of this but are unable to cover the user of headers.

      1. Apply the attached patch: 

        git am 0001-PATCH-1-1-DO-NOT-COMMIT-This-is-for-testing-only.patch

      2. Login as admin
      3. Purge caches
      4. Open browser developer tools and switch to Network tab
      5. Filter on "Document"
      6. Open any page on the site
      7. Select the request
      8. Look for the "Response Headers"
        1. Confirm that there was a header: X-Clacks-Overhead: GNU Terry Pratchett
