Add qbank_usage to core

      Upgrade

      1. On an existing Moodle instance with courses, upgrade your instance with the patch applied.
      2. Confirm that the upgrade completes successfully.
      Fresh installation
      1. Create a fresh install of Moodle with this patch.
      2. Confirm that installation completes successfully.
        Test scenario:
      • Create a new quiz.
      • Add some questions to the quiz.
      • Attempt the quiz.
      • Navigate to the created course: Actions menu (Gear icon) > Question banks
      • Confirm:
        • That the "Usage" column has "1" for all the questions added to the quiz.
        • That the usage count "1" is clickable
      • Click "1" on that row.
      • Conform:
        • That the question shows the quiz and course where its added.
        • That the attempt count matches the number of attempts by different users.

      Create a plugin to view the column to show the usage of the question in different quizzes.
      The number in the usage column should be clickable link, which should pop up a page where it will show the question and also a table of content to view where this question is used, previewquestion plugin and moodle table_sql can be a reference for this issue.
