Manage files dialog in editor has no explanation why 'submit' button is missing

      1. Add a label to a course.
      2. In the editor (TinyMCE or ATTO), click on 'Manage files' button.
      3. Observe that there is only a drag and drop box and nothing else, no way to insert the file after its uploaded.
      4. Upload a file.
      5. Close the dialog.
      6. Select 'Manage files' icon again and observe a message to the effect of 'unattached files' detected.

      When using the 'Manage files' button in an editor (TinyMCE or ATTO) there is no 'submit' button etc. to insert the uploaded file into the label / post etc. Please see attached screen shot.

      Tested in a label on own local site (Clean theme), iMoot 2015 site (More theme) and on a forum post on moodle.org (bespoke theme).
