Javascript error when course overview block was accidentally added twice

      When editing the "Default My home page" (Appearance->Default My home page) adding a duplicate block causes a JavaScript error.

      Uncaught TypeError: Cannot read property 'setAttribute' of null module.js:1

      This also causes the ability to click on the action button to be removed (i.e. you can't remove the duplicate block or any other block).

      This condition disappears when the duplicate database entry is manually removed from mdl_block_instances.
