Additional names section missing from the edit profile page

      1. Go to [Administration ► Site administration ► Users ► Permissions ► User policies].
      2. Change "Full name format" to include all of the place-holders apart from one.
      3. Go to [Administration ► My profile settings ► Edit profile] and check that the one name field not included in the setting is located under "Additional name".
      4. Go back to [Administration ► Site administration ► Users ► Permissions ► User policies].
      5. Change "Full name format" to include all of the place-holders.
      6. Go back to [Administration ► My profile settings ► Edit profile] and check that there is no heading "Additional name".
      7. Try a few other configurations and check that the "Additional names" section appears with the correct disabled name fields.

      1. In Administration > Site administration > Users > Permissions > User policies set full name format to something including the placeholder firstnamephonetic (but not using ALL available placeholders)
      2. Check edit profile page

      Expected result: Edit profile page includes an Additional names section containing settings not used in the full name format

      Actual result: Additional names section missing from the edit profile page
