If you go to a course and to the question bank the breadcrumb is something like this...

Both "Question bank" and "Questions" link to exactly the same place. In this case its /question/edit.php?courseid=2&cat=1%2C15

Similarly in the course admin block both "Question bank" and "Questions" link to the same place.

Could we do away with one as they seem redundant? Perhaps just have

Question bank
-Categories
-Import
-Export
