Start Google Chrome and go to the http://qa.moodle.net/mod/quiz/edit.php?cmid=34&cat=2,10&qpage=0&recurse=1&qbanktool=0&reordertool=0 as a teacher and press help icon "The basic ideas of quiz-making". You'll see that some elements (grade controls and delete icon) from the question are showed over help string.

Grade controls under some circumstances could also almost fully overlap edit icon for the question in quiz.

I tried to test this page on somewhat old IE7, but it was completely unusable there.
