The question bank "History" action item tries to use "t/log" as the icon, but this doesn't exist in Moodle's fontawesome icon mapping, resulting in fallback to image file and subsequent misalignment in the action menu:
Only seems to be a problem from 5.0 (alignment in 4.5 looks OK) - edit it's because MDL-83933 changed the width of all icons, introducing the misalignment here because the icon is now wider
