Accessibility issues on the feedback templates page

      To reproduce:

      1. Log in as an editing teacher in a course.
      2. Create a feedback activity in that course.
      3. Go to the Questions tab and add some questions.
      4. Open the Actions menu, select Save as template.
      5. Add a name for the template. Ensure that "Available for all courses" is not ticked. Save the changes.
      6. Save the questions again as a template, but this time, ensure that "Available for all courses" is selected.
      7. Go to the Templates tab.
      8. Run an accessibility checker, such as Axe DevTools or WAVE, on the page.
      9. Expected: There are no accessibility issues related to missing table headers and skipped headings.
      10. Actual: The actions column is missing a column header, and there are skipped page heading level issues.
