To reproduce:

Log in as an editing teacher in a course.
Create a feedback activity in that course.
Go to the Questions tab and add some questions.
Open the Actions menu, select Save as template.
Add a name for the template. Ensure that "Available for all courses" is not ticked. Save the changes.
Save the questions again as a template, but this time, ensure that "Available for all courses" is selected.
Go to the Templates tab.
Run an accessibility checker, such as Axe DevTools or WAVE, on the page.
Expected: There are no accessibility issues related to missing table headers and skipped headings.
Actual: The actions column is missing a column header, and there are skipped page heading level issues.
