1. Go to the question bank on a course
2. Click "Create a new question"

• Confirm the icons in the "Choose a question type to add" modal are square and not distorted as shown the description. It should be 16x16px.

3. Select a question type and add a question to the bank.

• Confirm the icon in the question list is also 16px square

4. Add a quiz to the course.
5. Go to the Questions tab and add a question from the question bank.
6. Switch to the question bank where you added the question above.

• Confirm that the icon in the question bank modal is also 16px square
