UX · Review the badge creation form, to reorganise the fields and remove/improve tooltips

      In the badge creation form, the author fields (name, email and URL) will be removed because OBv3.0 doesn't support them anymore:

       

      That's a good opportunity to reorganise the fields in this form (if required), remove/improve tooltips and suggest any other minor improvement related to this form.
