Set up:

1. Log in as admin
2. Create a Quiz activity with a completion setting(set completion date or not set on this step).
3. Log in as a student and complete the Quiz
4. Login as Admin > Navigate to Report > Activity completion
5. Click on Download in spreadsheet format or Download in Excel-compatible format (.csv)

Expected:

The file download should have the heading date/time: "Completed" when it have set completion date or not set.
