Hello,

When you fail a scorm activity, through course format and through the activity completion report, a check mark appears instead of a cross.

Here the scorm is set at the number of attempts to '1 attempt' and with 'Unlimited attempts' we don't have this problem.
The selected activity completion, 'Required statis' is 'Completed'.

Additional information:

After you make an attempt, sometimes the checkmark comes later or only when the scorm activity is saved with any modification (For example: Name) ore without modification after there is already an attempt.

With a test, for example, this goes well.
