When you fail to authenticate with MFA enabled you are shown a warning message with a button to logout. This workflow needs to be improved.

Where the original authentication method uses the Moodle login page (auth_manual, auth_db, etc) then this should follow the same pattern as if a username or password was incorrect, that is you are presented with the login screen and an error message describing what has occurred

For authentication methods that don't support the Moodle login page (auth_oidc etc.) then the message should be displayed on a separate page as it is now.

Both the error message and text on the "log out" button will need ux review.
