Have the name of a module configuration/type displayed in the title on module add/edit pages.

      Some modules allow you to create different configurations of the module and add instances of that configuration to a course. For example, the external tool module can have many tools created, with the user being able to select one of these tool from the activity chooser.

      The add/edit page displays the name of the module plugin, but gives no information about what configuration you are actually editing. The title displayed should include the name of the configuration.

      For example, if we have an external tool named 'Adjust Data', then the add module page should be titled 'Adding a new External tool - Adjust Data', or something similar. (It currently just says 'Adding a new External tool'). The edit page should be similarly titled.
