Hi, all,

In MoodleCloud (M4.1.2) and Sandbox demo (M4.2), all three types of surveys: Critical incidents, COLLES (actual, preferred and preferred&actual) and ATTLS, have no instructions anymore as they used to in M<3.11.

What to do:

In a course, add a survey activity (no matter what type) and click Save and display.

What is expected

You will see the survey with instructions for students.

What happens:

You see the survey with no instructions at all.

I add images of the instructions shown in M<3.11 for all surveys types and a couple of images of COLLES (preferred&actual) Survey in M4.1 and M4.2 with no instructions.
