Duplicated heading in assignment grader

      Replication steps

      1. Create a course with some students and an assignment
      2. View the assignment
      3. Press the "Grade" button
      4. The "Grade" heading in the pane to the right is displayed twice (screenshot attached).

      This appears to be due to MDL-72737. The new element-header template template add a second header.

      So far I've only seen this problem in the assignment module.
