      This tracker originates from MDL-74427 but aimed to fix bad data in question bank.

      Some old Moodle site might have bad data in their question bank, eg:

      1. Questions that have parent field set to an id that doesn't exist or
      2. Questions that have parent field set to an id that belongs to a different question category or
      3. Questions that are linked to a category that doesn't exist.
      4. Questions that reference a previous course (MDL-77625)
