Create a new course

Select More > Question bank from secondary navigation

Select Categories from tertiary navigation

Press any of the Default for <category> links (e.g. for course, category, system)

Confirm they load the question bank for each category

For example, my own dev site has the following:

$CFG->wwwroot   = 'http://moodle.internal/master';

See the following screenshot, clicking the Miscellaneous category leads to an incorrect URL

Note the path is duplicated, leads to a 404
