On a course with a quiz, some questions and therefore a question bank, observe that the 'Actions' column header and 'Edit' dropdown toggle do not overflow into the 'Created by' column.

When viewing the 'Question bank' in languages other than English (en) where the words 'Actions' and 'Edit' are bigger than the allocated width:

#categoryquestions .editmenu { width: 5em; }

then they can overflow into the next column.
