Flag question flashes from checkbox to icon as the page loads

      1. Go to a quiz.
      2. Start an attempt.
      3. While looking closely at the question flag icon, reload the page by putting the cursor in the URL bar and pressing enter..
      4. Verify that there is no flicker as the page loads.

      Note: if you reload the page by pressing F5, then the icon will flicker becuase Font Awesome is slow to load. That is not what this issue is about.

      Go to a page of a quiz (or anywhere you can see a question, e.g. https://qa.moodledemo.net/mod/quiz/view.php?id=32). Reload the page, and watch the flag icon closely. It starts as a checkbox, then changes to an icon as the page loads.

      This used to work nicely before Moodle 2.1! Re-fixing.
