There was a bandage fix for this problem addressed in ~~MDL-63260~~. But the solution still leads to duplicate random questions being created in the first place.

The preferred solution would be to not create a duplicate random question entry in the first place rather than have the duplicate entries be deleted later on by a scheduled task.
