Import a backup file, choose the attached mbz with a feedback activity, Continue

Restore into this course → Merge the backup course into this course, Continue

Next, Next, Perform restore, Continue

Click the "Whatever feedback"

Go to the "Edit questions" tab

Cross check there is a padding between the two form columns.

The choice of bootstrap grid elements is not correct which produces a "broken" view.

Not directly broken, but very narrow and not clean. Changing grid classes in the template (cols) would solve this problem.
