While editing lessons using Expanded mode, the interface doesn't consistently show the page title and the action icons for a page after the page has been updated.

Steps to reproduce:

1. Edit a Lesson
2. Select Expanded mode if currently viewing Collapsed mode
3. Click the Update "gear" icon to edit a page in the lesson
4. Click Save or Cancel from the page settings

The outcome of this is that the Expanded view of the lesson is shown again, but the updated page no longer has the region above the page contents that previously contained the page title and the action icons.  (They were there before...the Update "gear" icon is among the missing.)

This can also happen to the previous page when a new page has been added (I did this on the demo site):

1. Create a Lesson with a name and description
2. Select Expanded mode if currently viewing Collapsed mode
3. Create a page with a title, content, and a Next page continue Content label
4. Create a second page after the first, also with title, content, and a Next page continue Content label
5. (the first page should still have its title and action icons, but it doesn't.)

The user editing the lesson is in the Manager and Teacher roles for the course, and is also in the system Course Creator role.  I have also reproduced this with the system admin user, however.
