Lesson should use page headings instead of first fieldset to denote user action

      As noted in 49484 review, using the first fieldset on the page as a means to convey the action to the user is non-standard and we should change this so it's more in line with what we do elsewhere in Moodle.

      The page already has a header, which at the moment just contains the name of the lesson at all times. I think with some minor refactoring, we should be able to use this header instead of the fieldset method, similar to how quiz does this when editing a question.
