      1. In a given course with a quiz.
      2. Turn edit on and visit edit quiz page.
      3. Make sure the slot number is not misplaced (see screenshot)
      4. Turn edit off and go back to edit quiz page.
      5. Make sure the slot number is not misplaced
      6. Repeat the test on clean.

      You can use the attached quiz restore to avoid creating questions
