Just came across this while finalizing grades. Normally I have been just going to the list of assignments filtered by "Requires grading" turned on. I had some students complain that some of their assignments were not graded. I started just clicking on the assignments and just looking at the "Grading summary" which I know is easier and have come accross students who do not show up as "Graded" in the summary yet when I look at the assignments with the filter turned on they do not show up.
I'm not sure if this has been noticed before. As far as duplicating I can go to my previous backup and see if problem shows up, if needed. I can't send the backup because of confidentially requirements. Let me know if this is not being worked on and you need additional help from my end.
