Was able to reproduce this on qa.moodle.net
Steps to reproduce:
0. Set Debugging to developer
As a teacher:
1. Create a lesson activity
2. Add a matching question with the following settings:
Name: Fruit
Page contents: Match the fruit with their pictures
Matching Pair 1 Answer: Add ONLY a picture of an apple (I had added it from the wikimedia repository)
Matching Pair 1 Matches with Answer: Apple
Matching Pair 2 Answer: Add ONLY a picture of an pear (I had added it from the wikimedia repository)
Matching Pair 2 Matches with Answer: Pear
3. Save the question
4. Re-edit the matching question named "fruit"
DO NOT CHANGE ANY THING ON THE QUESTION
5. Click "Save Page"
