This follows on from ~~MDL-48373~~ We want to be able to say something llike:

And the following questions are in quiz "Quiz 1"

  | TF1 | 1 | 3.0 |

  | TF2 | 2 | 3.0 |

That is, question TF1 on page 1 of the quiz, and question TF2 on page 2, both marked out of 3.0.
