Remove links to assignment upgrade tool from admin menu.

      In 2.7 we forced assignments to use the new mod_assign. People had the option of replacing the code to keep running the old one - but time is up (really). I think we should remove the assignment upgrade tool from core (maybe we can stick it in plugins).
