Duplicate 'Identifier' header in CSV file when downloading Grading Worksheet

      1. In a give course with enrolled students, create an assignment, expand Feedback types and enable Offline grading worksheet.
      2. Go to the assignment and click on View/grade all submissions.
      3. Under Grading action dropdown, choose Download Grading Worksheet
      4. Open the file and make sure the column Identifier is not duplicated on CSV file.

      When downloading the Grading Worksheet for assignments, the Identifier header is repeated after the Email address header, causing the data rows to no longer align with their correct headers.

      This in turn causes importing the grades back into Moodle to detect no changes and not import.

      Example:

      Steps to reproduce:

      1. Create a course (with students)
      2. Add a gradeable item to that course (I've been using assignment)
      3. Edit assignment
      4. Under Feedback Types turn on Offline Grading Worksheet
      5. Go to the gradeable item and click on View/grade all submissions
      6. Under grading action choose Download Grading Worksheet
