After update from 2.5.1.2 to 2.6.2, can't duplicate quizzes

      Recently updated Moodle site from 2.5.1.2 to 2.6.2. Tried to duplicate numerous previously created quizzes. Moodle hangs up with wait symbol, then get error messages:

      error/error_question_answers_missing_in_db
