Questions are in categories at system-level, as they are shared among all courses
Questions become duplicated, but not every day and not every question
We create template courses, back them up, then restore for new courses
Clean question bank last week; watched every day for one week with no dup's
Restored eight template courses yesterday, and each question bank category shows many (but not all) dup's
Additional note: the quizzes that are being restored from backup template are all randomized questions... pulling random questions from a particular question category.
Also noticed that they are getting duplicated in other question categories where there are NO randomized quizzes. Sorry for assuming they might be related.
