System setting glossary_entbypage does not apply when creating a new Glossary activity

      Creating a new Glossary activity initiate with a fixed "10 entries per page" and does not use the Glossary's glossary_entbypage system setting.

      Suggested fix:

      $mform->setDefault('entbypage', $CFG->glossary_entbypage);
