Make doctonewwindow = true the default setting

      Until Moodle 2.3, the doctonewwindow was extremely inefficient and caused browsers to hang.

      Opening help links in your current browser window is really poor from a usability perspective and is not what users are used to.

      The performance issues are now fixed, and I'd like to suggest that we should either phase out the option entirely, or at least set it to default to true.

      I'm happy to do the necessary work, but which route do we feel it best to go down?

      As well as enabling doctonewwindow by default, let's improve the 'Help and documentation' link by adding an icon to indicate it will open in a new tab (as for the 'Services and support' link).
