      1/ make sure enrol_self setting for adding of instances to new courses is enabled
      2/ add new course and make sure the self method was added with requested default settings
      3/ change the default enrol_self settings and add new instance into existing course - make sure the default settings are present in the form

      Also when adding new instances manually they should be enabled by default no matter what the default setting is for instance status because that is intended for automatically created instances...

      I am going to use this issue for two issues because the testing will be done only once and the code changes are in the same area...

      OOPS, there are problems with the defaults too.
