Maximum number of attempts setting in the lesson module does not appear to work.

I've tried a number of combinations of the settings without any joy. The settings below should work but over three attempts are allowed on each question.

Test settings:

Practice lesson - No
Re-takes allowed - No
Allow student review - No
Maximum number of attempts - 3
Provide option to try a question again - Yes
