There appears to be a bug within Quiz when using Embedded Answer (Cloze) Questions. On multiple servers, we have been able to somewhat replicate the issue that is being experienced by the person who first reported it to us. Due to this issue, it is believed that it is causing the other issue for already created Embedded Answer (Cloze) Questions. If you try to edit a pre-exsisting Embedded Answer (Cloze) Question you get a very similar occurrence. You receive the usual edit question screen, but then the question never appears, and you cannot edit. See this image: http://www.screencast.com/t/DtuqcBZaf
