Add label for select and text input elements in quiz

      1. As teacher, go to a course and create a quiz
      2. Add few questions to quiz and two description questions
      3. click edit quiz.
      4. Select "Order and paging" tab and check the "Move selected questions to page:" (on top and bottom) text elements. Make sure it has a proper label
      5. check question order input elements (next to up/down arrow icons). Make sure it has a proper label
      6. On "Editing quiz" tab, check the "Marked out of" text elements, they SHOULD have a proper label
      7. In Navigation block under the quiz, select Results -> Statistics.
      8. check the element called "menudownload", it should have a proper label

      There are several select input fields that do not have labels explicitly tied to them. Often this is because there is a visual cue as to what information is being asked for but the visual cue is not explicitly linked to the input element.

      Because this problem is sporadic it might be necessary to break this task out into smaller sub-tasks for each instance of the problem.
