      1. As an admin, enable the feedback module going to the admin tree -> plugins -> activity modules -> Manage activities
      2. As an admin, go to a course, add a feedback instance and view the instance
      3. As an admin, add a "multichoice" question with the "Multiple choice - single answer allowed (dropdownlist)" option of "Multiple choice type" selected
      4. As an admin, add a "multichoice (rated)" question with the "Multiple choice - single answer allowed (dropdownlist)" option of "Multiple choice type" selected
      5. As an admin, return to the edit questions tab of the feedback instance, there SHOULD be labels with the accesshide class in both select elements
      6. As a course student, fill the feedback instance and logout
      7. As an admin, go to the feedback instance, copy the course module id and replace the browser URL with /mod/feedback/analysis_course.php?id=$CMID (I don't know another way to access this script)
      8. As an admin, check the search course text input there SHOULD be a label

      There are several select input fields that do not have labels explicitly tied to them. Often this is because there is a visual cue as to what information is being asked for but the visual cue is not explicitly linked to the input element.

      Because this problem is sporadic it might be necessary to break this task out into smaller sub-tasks for each instance of the problem.
