SCORM report does not close 2 div tags and messes up theme on report with no attempts

      1. Set the site's theme to afterburner
      2. Create a SCORM activity
      3. Navigate to the SCORM Activity
      4. Click the Report tab

      The report page doesn't close a div tag and the form tag is a user can delete and there are not attempts. The report also doen't close a second div tag if there are no attempts.

      This causes Moodle themes to not display correctly and the user has difficulty exiting.

      Attached is a patch that resolves the issue by adding closing tags to the else statement after checking it there are attempts.
