Im unable to add a question directly to a quiz activity ie not from the question bank. Error, debug info below. Steps to replicate issue:

Login as editing teacher to a course
Editing on
Add an activity > Quiz > Add a Question... > Choose a question type to add > Next

Neither a question id, nor a category id and question type, was specified.
