Record start time for enrolments

      Adding a column to record when a user was enrolled in a course, or if specified - when there enrollment started.

      In the table user_enrolments

      Naming the column timeenrolled, as timecreated was ambiguous, and timestarted was too similar to the existing column timestart.

      This patch requires a db upgrade.
