Navigation broken in Quiz entry screen.

      1. Create a new quiz.
      2. Leave most options as default, randomise questions and switch off adaptive mode.
      3. Save and display and get http://screencast.com/t/MTI0NTBkMjE - note blocks being duplicated on LHS.

      Browser is FF3 running on qa Moodle site.
