Site policies: Allow site setting for maximum number of login attempts

      1. Apply the patch
      2. Login as admin and make sure can set maxlogin setting (Settings -> Site administration -> Plugins -> Authentication -> Manage authentication)
      3. Set this to 2
      4. Logout and try 2 invalid logins
      5. On 3rd login you should get max login error.

      I was surprised to see that $max_logins in /lib/moodlelib.php is hardcoded to 10.

      I think it would be a nice option to add this to site policies (http://localhost/moodle/19stable/admin/settings.php?section=sitepolicies)

      I'm attaching a quick patch (created against 1.9) that implements what I'm thinking.
