      There are several common problems that affect stability of moodle, some of the problems may be detected on the fly and fixed - some are more complex.
      This is a meta bug - file each issue as new subtask.

      Usual causes are:

      • concurrent db access
      • double clicking when submitting forms and interrupting page loading
      • coding errors
