Hi there

I am reporting an issue concerning mod_questionnaire. It handels labels and the numbering of questions incorecctly in the app:

1. open a course
2. create a questionnaire activity
3. add a question with type "label"
4. add a question with any other type, e.g. Yes/No
5. open the questionnaire activity in a browser (the label and the question are displayed as expected)
6. open the questionnaire activity in the app (the label is invisible and the numbering of the question is "2" instead of "1")

Tested with app version 4.2 and 4.3, android and ios, LMS 4.2 and 4.3.
