When try to join a questionnaire module on any course, the name item is replicated on the module menu.

Scope of Issue

The Questionnaire activity name is showed two times when join into the activity module.

Steps to Replicate/Observe

1. Create any Questionnaire module activity or get into a module already created.

2. Type any name to the activity.

3. Save and display activity.

4. You should be able to see two times the same activity name.

OBSERVED BEHAVIOR

There are two times the same activity questionnaire name.

EXPECTED BEHAVIOR

The questionnaire activity name should be displayed just one item name.

Any conditions not detailed above which are necessary to replicate (settings that must be enabled) etc.

This issue is replicated on Moodle and OpenLMS 4.0 sites.

On 3.11 sites are working well.

It could be possible to hide a duplicated name with CSS.
