i am using moodle 3.6 for conducting exams and other graded activities. in our organizational setup, we follow a certain chain for approval of exam questions before conducting exam. in our traditional setup the instructor creates the quiz, forwards it to Senior Instructor (SI) then SI vets the questions and forwards to Chief Instructor (CI). the CI is final approval authority. after approval the exam is conducted.

as far as moodle is concerned, we get approval of questions from the above mentioned chain and then import questions to moodle quiz.

It would be very productive if such approval chain may be implemented within moodle to make the process easier.

it is pertinent to mention that i have tried the plugin named "Workflow Block". but that does not serve the purpose.
