Currently, in the Activities block, Questionnaires displays a list of a course's questionnaires with 3 columns: Name (and link to questionnaire), Summary and Questionnaire type. This display is exactly the same for Teacher and Student, including the Questionnaire Type information, which is useless to students.
Plus when this list of questionnaires is displayed, the navigation etc. blocks are not displayed.
I have decided to make this display more "standard", i.e. look more like the display of other standard activities.

I have completely re-written questionnaire/index.php script, basing it in quiz/index.php.

The attached screenshots show BEFORE and AFTER displays.

I will shortly commit a related version to the remotelearner github. The changes can be viewed on my moodle 2.4 test site at http://rezeau.info/moodle24/

Looking forward to comments before committing to questionnaire version 2.4.
